Forms
Forms let you collect information from clients in a structured way: an initial assessment, a weekly check-in, an energy rating, or a satisfaction survey. You prepare the questions once, choose the day each client should answer them, and TrainerStudio keeps the history.
The workflow is always the same:
- Create the form in your library.
- Add it to the client's plan on the date when you want it to appear.
- The client answers it in their app and submits it.
- Review the responses from their profile and, if the form is recurring, compare their progress.

Build the questions on the left and check on the right how the form will look to your client.
Two common uses

- Run recurring check-ins: schedule the same form every week to compare energy, sleep, adherence, or discomfort over time.

Short forms usually work best. For a weekly check-in, start with four or five questions that you will actually use when adjusting the plan.
