Track your clients with forms

Before designing a client's first training plan you need information: injuries, goals, previous experience, available time. With TrainerStudio's forms you can collect all of this in a structured way without relying on WhatsApp messages or scattered emails.

Create a form

  1. Go to Library > Forms.
  2. Click Create new form.
  3. Give it a clear name, for example "Initial Assessment" or "Monthly Check-in".
  4. Add the questions you need.

Each question is free text, so the client can respond with as much detail as needed. A good initial assessment form might include questions like:

  • Do you have any current injuries or discomfort?
  • What is your main goal?
  • How many days per week can you train?
  • How long have you been training?
  • Do you follow a specific diet or have any dietary restrictions?
  • Is there any exercise you cannot or do not want to do?

You don't need to include twenty questions. With 5–7 well-chosen questions you have enough to design a first plan.

Assign the form to a client

  1. Go to the client's profile.
  2. Assign the form you created.

The client will receive the form in their app and can fill it in at any time. Once submitted, you can view the responses in two ways.

How your client sees it

The process for the client is very simple: they open the corresponding day in the calendar, tap on the pending form, answer the questions, and hit submit. The status automatically changes to completed.

View the responses

From the client's plan: completed forms appear expanded directly in the weekly calendar, showing questions and answers without needing to click. Useful for quickly reviewing responses while planning.

From the Form Responses section: in the client's side menu, click "Form Responses". Here you have the complete history of all responses, organized by date. You can filter by form, status, or date range. Click any response to view the details.

Reuse your forms

Forms are saved in your library and can be assigned to as many clients as you like. This is especially useful if:

  • You want all new clients to go through the same initial assessment.
  • You run periodic check-ins (monthly or quarterly) with a standard form.
  • You send satisfaction surveys to improve your service.

A good initial assessment form saves you time and gives you a much more complete picture of the client than an informal conversation. Everything is also on record: you can go back and review it months later if you need to revisit their history.
